EXPANDING A CHW LED DIABETES INTERVENTION

Funding supports a community-based project to reduce the burden of type 2 diabetes mellitus (T2DM) through expanded outreach and education led by community health workers (CHWs). The project delivers On the Road to Living Well with Diabetes (OTR), an evidence-based intervention developed by the Joslin Diabetes Center, using bilingual, culturally responsive materials in small-group settings. Trained CHWs provide education focused on healthy hemoglobin A1c (HbA1c) management, lifestyle change, and navigation to healthcare providers and local resources. Implementation of this project targets delivery of OTR to 1,250 individuals diagnosed with T2DM or pre-diabetes who have limited access to diabetes education across the Paso del Norte region. Prior implementation has reached more than 1,243 participants in El Paso County, southern Doña Ana County, and Ciudad Juárez, Chihuahua, with an additional 1,000 participants anticipated by August 31, 2025. Project activities serve communities throughout El Paso County; southern Doña Ana County, including Sunland Park, Santa Teresa, Mesquite, Vado, Chaparral, San Miguel, and Mesilla; and areas of Ciudad Juárez, Mexico, strengthening access to diabetes education and support across the border region.
