The objective of the proposed project is to mitigate the deleterious effects of diabetes and to improve health outcomes by preventing the onset of diabetes and reducing the severity of this chronic disease. This will be accomplished through the implementation of "On the Road to Living Well with Diabetes", a community health worker (CHW) led evidence- based intervention that provides culturally sensitive, bilingual educational materials, and navigation to resources, which afford participants the opportunity to self-manage diabetes. The goal is to expand and market diabetes education and awareness to 900 diabetics and/or people at high risk for developing diabetes in areas of the Paso del Norte region in need of diabetes education. The five-week "On the Road to Living Well with Diabetes" small-group (10 or less participants) intervention, has been successfully utilized by New Mexico State University and adopted by the University of Texas at El Paso Border Biomedical Research Center as a viable and easy to follow educational diabetes program delivered by trusted, certified, and trained CHWs.
